One of the benefits that club
members have is ordering E-CAPS and HAMMER NUTRITION products at discounted prices.
Periodically throughout the year we will place orders. Instructions If you have Excel on your
computer you can use this spreadsheet
to fill out your order. Simply enter the quantity of each item that you want
and the spreadsheet does the rest for you. Don’t forget to enter your name,
address and phone number on the top of the form. Print it and you are done! If you do not have Excel then
print this file out
and you will need to fill in all the info by hand. Once you complete either
version of the form mail the form along with your check for the total amount owed
